Job summary

The West Side Federation for Senior and Supportive Housing is seeking a Social Worker II in New York. The role collaborates with the clinical team to provide comprehensive social work services to residents and supports 1:1 relationships to promote independent living.

Applicants should hold an MSW (or MA in a human services field with at least two years of social work experience) and be fluent in English and Spanish.

Qualifications

• MSW preferred; or MA in human services with at least two years of social work experience.
• Fluency in written and oral English and Spanish required
• Experience with older adults and mental illness/substance use preferred

Responsibilities

• Assist residents in obtaining and maintaining medical and mental health services.
• Assist residents in obtaining and maintaining entitlements and home care.
• Provide care coordination between community service providers and residents.
• Assist residents with budgeting, money management, and rent payments.
