Job summary

Transitions Hospice is seeking a Licensed Social Worker to manage a meaningful caseload within our interdisciplinary team. You will assess patients and families, provide ongoing emotional support, and connect families with community resources while guiding Advanced Directives.

A CSWE-accredited MSW is required, with licensure and at least 2 years in healthcare or social services. We value compassionate, autonomous professionals who travel locally, with a schedule of Mon-Fri, part-time under 32

Qualifications

• Master’s degree in Social Work from CSWE-accredited program
• Licensure/certification as required by the state
• Minimum 2+ years social service experience in healthcare or social services
• Hospice/palliative care experience preferred
• Comfort with grief, loss, and end-of-life conversations
• Strong knowledge of community resources and care coordination
• Excellent documentation skills and attention to compliance
• Self-directed with ability to work with little supervision
• Valid driver’s license and willingness to travel locally

Responsibilities

• Conduct initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers
• Provide ongoing emotional support and counseling throughout the care journey
• Identify and connect families with community resources and financial assistance
• Guide families through Advanced Directives with clarity
• Assist with facility placement when appropriate
• Maintain accurate clinical documentation in patient records
• Collaborate with interdisciplinary team in care planning
• Participate in on-call duties evenings and weekends
• Ensure compliance with local/state/federal laws and Medicare policies

Job description

Coverage Territory: Joliet, Chicago, Lake County and surrounding areas

Schedule: Mon- Friday (8 hr. shifts) Part-Time less than 32 hours a week

Salary Range: $35.00 - $38.00 an hour

What You'll Do

As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:

• Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers
• Providing ongoing emotional support and counseling as a consistent presence throughout the care journey
• Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs
• Guiding families through Advanced Directives with clarity and sensitivity
• Assisting with facility placement for patients when appropriate
• Maintaining accurate, timely clinical documentation at the patient's residence and in agency records
• Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans
• Participating in on-call duties on evenings and weekends as needed
• Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures

What You Bring

• Master’s degree from school accredited by the Council on Social Work Education required in states as specified.
• Licensure and/or certification as required by the State.
• Minimum of 2+ years social service experience in a health care setting or social services agency.
• Experience in hospice, palliative care, or a related healthcare setting preferred
