Job summary

Transitions Hospice - Illinois seeks a Licensed Social Worker to support patients and families in our Ottawa, Streator, Dwight, Kankakee coverage area. You will carry a meaningful caseload and advocate for psychosocial care throughout the hospice journey.

This role emphasizes grief support, community resource coordination, and collaboration with an interdisciplinary team. PRN as needed, travel locally, and ensure compliant documentation and compassionate care.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Licensure/certification as required by state.
• Minimum of 2+ years social service experience in health care or social services.
• Hospice/palliative care experience preferred.
• Comfort with grief, loss, and end-of-life conversations.
• Knowledge of community resources and care coordination.
• Excellent documentation and compliance skills.
• Self-directed with minimal supervision.
• Valid driver’s license and willingness to travel locally.

Responsibilities

• Conduct thorough psychosocial, spiritual, and pre-bereavement assessments for patients and families.
• Provide ongoing emotional support and counseling throughout the care journey.
• Identify and connect families with community resources and financial assistance.
• Guide families through Advanced Directives with clarity and sensitivity.
• Assist with facility placement for patients when appropriate.
• Maintain accurate clinical documentation at patient residence and in agency records.
• Actively contribute to interdisciplinary team and IDG meetings.
• Participate in on-call duties evenings and weekends as needed.
• Ensure compliance with local, state, and federal laws and Medicare regulations.

What You'll Do

As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:

• Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers


• Providing ongoing emotional support and counseling as a consistent presence throughout the care journey


• Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs


• Guiding families through Advanced Directives with clarity and sensitivity


• Assisting with facility placement for patients when appropriate


• Maintaining accurate, timely clinical documentation at the patient's residence and in agency records


• Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans


• Participating in on-call duties on evenings and weekends as needed


• Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures

What You Bring

• Master’s degree from school accredited by the Council on Social Work Education required in states as specified.


• Licensure and/or certification as required by the State.


• Minimum of 2+ years social service experience in a health care setting or social services agency.


• Experience in hospice, palliative care, or a related healthcare setting preferred


• Deep comfort with grief, loss, and end-of-life conversations


• Strong knowledge of community resources and care coordination


• Excellent documentation skills and attention to compliance


• Self-directed with the ability to work with little supervision


• Valid driver’s license and willingness to travel locally for patient visits
