Job summary

JourneyCare Hospice is seeking a dedicated Hospice Social Worker to join our growing team in Oak Park, IL. You will perform psychosocial assessments, counsel families, and coordinate access to benefits and community resources.

The role offers competitive salaries, generous time off, 401(k) with match, and a supportive team culture. Monday–Friday 8:30 AM–5:00 PM with after-hours/on-call requirements.

Qualifications

• Social Work Degree required.
• MSW is required.
• Current State Licensure in Social Work is required.
• Minimum of one year of experience in health care or hospice setting.
• Valid driver’s license and proof of insurance required.
• Bi-lingual (English/Spanish) preferred.

Responsibilities

• Performs psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures needs are addressed in the plan of care.
• Provides exceptional service to patients, families, referral sources, co-workers, and other customers.
• Counsels patient and family/caregivers and staff regarding stress and coping difficulties.
• Provides accurate and timely documentation of patient services to reflect the plan of care.
• Coordinates applications for SSI, Hospice, Medicaid, and Medicaid Waiver and connects with community resources as needed.
• Acts as liaison between patients/families and community agencies.

What You’ll Do

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Salary Range: $62,000-$80,000 - Offer based on experience
