Transitions Hospice is seeking a Licensed Social Worker to provide psychosocial support for patients and families across the hospice journey. You will conduct assessments, offer ongoing counseling, and connect families with community resources while ensuring compliance and meticulous documentation.
You will collaborate with an interdisciplinary team and participate in on-call coverage, with a focus on compassionate, patient-centered care in the Marshall, Mattoon area and surrounding regions.

Schedule: M-F; 8am - 5pm
