Transitions Hospice is seeking a Licensed Social Worker to provide compassionate psychosocial support to patients and families within the hospice journey. You will assess, counsel, and connect families with community resources while maintaining accurate documentation and coordinating with an interdisciplinary team.
The role requires a social work degree, state licensure, and at least two years in health care or social services. Travel to local patient sites will be routine.
Coverage Territory: Bay, Saginaw, Arenac, Midland Counties and surrounding areas
Schedule: Mon-Fri 8am-5pm
