Job summary

Transitions Hospice - Michigan is seeking a Licensed Social Worker to join our hospice team in Kalamazoo and surrounding areas. You will manage a meaningful caseload, provide psychosocial support, and connect families with community resources and financial assistance throughout the hospice journey.

You will guide families through Advanced Directives, maintain documentation, and collaborate with an interdisciplinary team.

Qualifications

• Master’s degree from CSWE-accredited program.
• Licensure/certification as required by the state.
• Minimum 2+ years social service experience in healthcare or social services.
• Experience in hospice/palliative care preferred.
• Deep comfort with grief, loss, and end-of-life conversations.
• Strong knowledge of community resources and care coordination.
• Excellent documentation skills and attention to compliance.
• Self-directed with ability to work with little supervision.
• Valid driver's license and willingness to travel locally.

Responsibilities

• Conduct thorough psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers.
• Provide ongoing emotional support and counseling throughout the care journey.
• Identify and connect families with community resources and financial assistance.
• Guide families through Advanced Directives with clarity and sensitivity.
• Assist with facility placement for patients when appropriate.
• Maintain accurate clinical documentation at the patient’s residence and in agency records.
• Actively contribute to interdisciplinary team and IDG meetings to shape holistic care plans.
• Participate in on-call duties during evenings and weekends.
• Manage caseload while ensuring compliance with laws, Medicare regulations, and policies.

What You'll Do

As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:

• Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers
• Providing ongoing emotional support and counseling as a consistent presence throughout the care journey
• Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs
• Guiding families through Advanced Directives with clarity and sensitivity
• Assisting with facility placement for patients when appropriate
• Maintaining accurate, timely clinical documentation at the patient's residence and in agency records
• Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans
• Participating in on-call duties on evenings and weekends as needed
• Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures

What You Bring

• Master’s degree from school accredited by the Council on Social Work Education required in states as specified.
• Licensure and/or certification as required by the State.
• Minimum of 2+ years social service experience in a health care setting or social services agency.
