Job summary

JourneyCare Hospice in Vernon Hills, IL is seeking a Hospice Social Worker to join our growing team. This role conducts psychosocial assessments, develops plans of care, and coordinates community resources to support patients and families in their homes and hospice settings.

Responsibilities

• Performs psychosocial assessment of patients and families in the hospice program in a timely manner and ensures identified needs are addressed in the plan of care.
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers.
• Counsels patient and family/caregivers and staff regarding stress and coping difficulties.
• Provides accurate and timely documentation of patient services to reflect the plan of care.
• Assists with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver; connects resources from community agencies.
• Serves as liaison between patients/families and community agencies.

What You'll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient's plan of care. Provides an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Salary Range: $62,000-$80,000 - Offer based on experience
