Overview

The Home Services Social Worker identifies the psychosocial needs of patients and families through assessment. Social work interventions range from resource support identification and acquisition (including community support, financial and environmental enhancement) to short‑term counseling and emergent crisis intervention. Social Services are provided as part of a collaboration with interdisciplinary teams. Hours may vary depending upon census and program need.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.
• Develop a bereavement plan of care to address family member/caregiver needs.
• Interpret and communicate patient/family faith and cultural traditions; identify and communicate when spiritual/religious beliefs may impact the physical and psychosocial care provided by other team members.
• Educate patient and family members in a manner that overcomes barriers; match their learning capabilities and meet fundamental needs.
• Complete visit notes and orders in a timely manner according to policy; correct care plans as required within the specified timeline.
• Document psychosocial patient/family assessments, financial assessment, and MSW interventions within patients’ electronic medical record.
• Implement social work plans that result in enhanced family system strength, increased utilization of community resources by patient/family/caregiver, maximization of medical benefits, improved environment for care delivery, dignity for the dying patient, maximized coping skills, and support for cultural beliefs; evaluate effectiveness and modify interventions as needed.
• Facilitate and support patient decisions and communications of self‑determined life care decisions.
• Provide care according to plan of care/orders and develop social work plan of care in collaboration with the interdisciplinary team.
• Practice sensitively to the needs of patients and families; demonstrate understanding of palliative/comfort philosophy and approach.
• Identify and respond to indicators of imminent death, addressing patient/family needs at time of death.
• Demonstrate understanding of Medicare Hospice Benefit including eligibility, admission, election, certification, recertification, transfer, non‑recertification, and revocation.
• Utilize Memorial Funds appropriately and submit documentation in a timely manner according to policy.
• Complete or assist patients/caregivers in advanced directives, including living will, HCPOA, and POLST forms.

Department Specific Job Functions

• Assist with transportation barriers.
• Assist with referrals for lack of access to food, clothing, and power bills.
• Make referrals for help in the home.
• Assist with applications (Medicaid, Community Care, SSDI).
• Make Elder Abuse/Neglect referrals.

Qualifications

• Educational Requirement: Master’s Degree in Social Work.
• Licensure/Certification Requirements:
  • Licensed Social Worker (LSW) – Illinois Department of Financial and Professional Regulation (IDFPR).
  • Or Licensed Clinical Social Worker (LCSW) – Illinois Department of Financial and Professional Regulation (IDFPR).
  • Driver’s License – Secretary of State (SOS).
  • Proof of auto insurance – required.
• Experience Requirement: 1+ years in Hospice.
• Specialized Knowledge and Skills: Ability to work as part of a team.

Salary Range

The compensation range for this position is $27.36 per hour – $45.69 per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered to a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
