The ideal candidate holds a Master's in Social Work and a Licensed Social Worker certificate, with at least one year of medical social work experience and strong communication skills; the team focuses on compassionate, quality end-of-life care.

Qualifications

• Master's degree in Social Work required.
• Licensed Social Worker (LSW) certification required.
• Minimum 1 year of medical social work experience; hospice experience preferred.
• Ability to multitask and prioritize workload.
• Must be willing to travel.
• Strong verbal and written communication skills.

Hours:

TheSocial Workerwill be responsible for but is not limited to:

1. Assesses emotional factors related to terminal illness.
2. Assesses environmental resources and obstacles to maintain safety
3. Provides emotional support, short-term crisis intervention and individual and/or family counseling when indicated
4. Maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
5. Evaluates patient/family response to psychosocial interventions
6. Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions
7. Performs comprehensive initial psychosocial assessments Assesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect

Education:
