JourneyCare Hospice is seeking a Hospice Social Worker in Vernon Hills, IL to perform psychosocial assessments and coordinate support within the hospice program. This role emphasizes timely care, compassionate service to patients and families, and effective collaboration with the care team.

Candidates must have an MSW, current Social Work licensure, and at least one year of healthcare or hospice experience. Salary range is competitive with time-off and benefit packages listed in the posting.

HOURS: Monday-Friday 8:30AM-5PM - On-Call Rotation Afterhours and Weekends

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Salary Range: $62,000-$80,000 - Offer based on experience
