Job summary

Pathways Hospice & Transition Home Healthcare is seeking a Social Worker for Home Health & Hospice services. This PRN position requires supporting the mission by addressing patients’ physical, mental, emotional, spiritual, and psycho-social needs with the care team.

The role involves admissions interviews, Do Not Resuscitate coordination, and policy development to enhance patient well-being. A Texas social work license and supervisory experience in hospice are required.

Qualifications

• Must possess a Bachelors or Masters degree in Social Work and be a licensed social worker in Texas.
• Minimum of two years of supervisory social work experience in hospice or related medical home health field.
• Outstanding organizational skills and professional interaction with patients, families, and staff.
• Proficient in computer systems, including Word, Excel, and Electronic Health Record systems.
• In good standing with Federal and State Government.

Responsibilities

• Plans, develops, organizes, implements, evaluates, and directs the social services program of the hospice patient.
• Interviews resident and family during admission to complete a social history and assess needs.
• Explains and obtains Advanced Directives with residents and families.
• Works with Nursing to ensure Do Not Resuscitate orders are accurate.
• Develops and implements policies for identifying social and emotional needs.
• Applies social work theory and ethics under supervision to restore or enhance functioning.
• Other duties as assigned.

The Job

Support the successful execution of our mission and Core Values by improving or maintaining each patients’ ability to control everyday physical, mental, emotional, spiritual and psycho-social needs.

Responsibilities

• Plans, develops, organizes implements, evaluates, and direct the social services program of the hospice patient
• Interviews resident and family during admission process to complete a social history and assessment of the patient to identify problem areas, and to develop rapport
• Responsible for explaining and obtaining Advanced Directives (Directive to Physician, Medical Power of Attorney) with residents and families.
• Works with Nursing department to ensure accurate Do Not Resuscitate orders are obtained for patients
• Develops and implements policies and procedures for identifying the medically-related social and emotional needs of the patient.
• With qualified supervision or with the appropriate license, applies Social Work Theory, knowledge, methods, ethics and the professional use of self to restore or enhance social, psychosocial, or biopsychosocial functioning of patients.
• Other duties as assigned

Must Haves

• Must possess a Bachelors or Masters degree in Social Work, must be a licensed social worker in the State of Texas.
• Minimum of two (2) years of social work experience in a supervisory capacity in a hospice or other related medical home health field
• Must have outstanding organizational skills and possess the ability to deal tactfully with associates, patients, family members, visitors, government agencies/personnel and the general public.
• Proficient in the use of computer systems (Word, Excel, Electronic Health Record system, etc.)
• Must be in good standing with the Federal and State Government
