Job summary

Pathways Hospice (Newest Start Up) is seeking a Registered Nurse (RN) - Hospice for PRN shifts covering Northwest, Southwest, and Southeast Houston service areas.

The role focuses on leading nursing care, collaborating with the care team, and delivering comprehensive home-based hospice services. The nurse will assess needs, develop care plans, educate families, and maintain clinical documentation. Night and weekend coverage are required per patient needs.

Qualifications

• Valid RN license in Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR is preferred but not required.
• Excellent communication and people skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and families on managing health conditions and medications.
• Prepare timely, complete, and accurate clinical notes.

Responsibilities:
