Job summary

Community Healthcare of Texas is seeking a Social Worker in Dallas, TX to provide therapeutic support, education, and resource information to patients and their families facing serious illness. You will advocate for patient needs within the care team and with families to honor patient wishes.

The role emphasizes counseling on end-of-life issues, coordination of the Plan of Care, and referrals to community resources.

Qualifications

• Master's degree in Social Work required.
• Reliable transportation and a valid driver's license.
• Hospice or healthcare experience preferred.
• Proficient skills using a computer are required.

Responsibilities

• Assess the psychosocial needs of the patient and family members

POSITION SUMMARY

To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.
