Job summary

Pathways Hospice (Newest Start Up) is seeking a Registered Nurse for PRN hospice care in the Houston area to deliver compassionate, high-quality patient experiences in home settings.

The RN will oversee nursing care teams, coordinate with physicians, educate families, perform patient assessments, and ensure timely charting. Night and weekend coverage may be required to meet patient needs and ensure continuity of care.

Qualifications

• Valid RN license in Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and compliance.
• Experience with HCHB EMR is a plus.
• Excellent communication and people skills.
• Ability to work independently and as part of a team.

Responsibilities

• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.

• Night and weekends are required per hospice needs of patients

Registered Nurse (RN) - Hospice

Shift Available: PRN

Service Areas for Coverage: Northwest, Southwest, Southeast

Responsibilities

• Oversee nursing care team.
