Job summary

Three Oaks Hospice is seeking a licensed social worker to provide coordinated hospice care in patients' homes in the San Antonio area. The role involves psychosocial assessments, family counseling, and coordination with the IDG/IDT to ensure compassionate, patient-centered care.

Ideal candidates will have an MSW or BS in social work and an LMSW license, plus knowledge of community resources and strong interpersonal skills. Reliable transportation and a valid driver's license are required.

Qualifications

  • Bachelor's or master's degree in social work from a State accredited program.
  • Current LMSW license to practice social work.
  • Working knowledge of community resources.

Responsibilities

  • Assess and complete psychosocial assessment within five days of hospice election.
  • Assist with advance directives, funeral planning, discharge planning, and family counseling as needed.
  • Provide patient and family/caregiver counseling around end-of-life issues.
  • Attend IDG/IDT meetings and coordinate with the team to plan patient care.
  • Promote hospice philosophy and maintain privacy and professional standards.
  • Follow infection control policy and participate in QAPI activities.

Position Summary

Primary function is to provide coordinated care in the homes to patients of all age groups and perform psychosocial assessments analysis counseling and referrals to meet the needs of the patientfamily

Essential Duties
  • Assesses and completes the Comprehensive Assessment within five days of the patients election of hospice care to identify patient and familys psychosocial financial environmental and community needs as evidence by POC documentation clinical records IDGIDT meetings and community resource referrals
  • Assists with advance directives funeral planning discharge planning family counseling and may assist in changes in the level of care as indicated by the patient or familys needs
  • Uses effective interpersonal communication skills both verbal and written
  • Meets mandatory continuing education requirements of the Hospice and licensing board
  • Demonstrates commitment professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements
  • Provides patient and familycare giver counseling around issues of end of life death and grief
  • Promote hospice philosophy and administrative policies to ensure quality of care
  • Attends IDGIDT meetings participate in the patient care planning process and coordinating and collaborating with theIDGIDT to promote coordination of patient care
  • Accepts clinical assignments that are consistent with education and competence to meet the needs of the patients
  • Provides an environment which promotes respect for patients and their privacy and property
  • Participates in the Hospices QAPI program
  • Demonstrates commitment professional growth and competency
  • Follows the Hospice infection control policy
  • Promotes the Hospices philosophy and administrative policies to ensure quality of care
  • Participates in the Hospices educational in service programs as require
QUALIFICATIONS
  • Bachelors or masters degree in social work from a State accredited program
  • If has a bachelors degree and is hired after 12022008 must be supervised by a Master of Social Work
  • Current and valid license as a LMSW
  • Working knowledge of community resources
  • Good interpersonal skills
  • Hepatitis profile
  • Current State Certification
  • Reliable transportation
  • Valid Drivers License and current auto liability insurance
