Job summary

Lakes Region VNA and Hospice is seeking a compassionate Social Worker to join our award-winning team serving Meredith, NH and surrounding areas. The role focuses on psychosocial assessment, care planning, and family support across hospice and home care settings.

Qualifications

• Master's degree in social work required.
• At least one year of healthcare experience, preferably hospice or home care.
• Current and valid Social Worker license in New Hampshire.

Responsibilities

• Conduct psychosocial assessments of patients’ needs and resources.
• Develop and implement individualized care plans with the team.
• Provide counseling and support to patients and families dealing with illness and end‑of‑life issues.
• Coordinate community resources, referrals, and financial assistance.
• Respond to crises and provide timely interventions.
• Maintain accurate documentation in the medical record.
• Collaborate with nurses, physicians, and other professionals.
• Educate patients and families about rights and available services.
• Provide hospice support and bereavement guidance.
