Job summary

Trinity Health At Home – Grand Haven is seeking a Hospice Medical Social Worker (MSW) to provide consultation and services to homebound patients, addressing social and emotional issues that interfere with medical regimens.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Michigan MSW license is required.
• At least 2 years of social work experience in a healthcare setting.
• Ability to serve acutely and chronically ill patients.
• Ability to complete written, verbal, and observational tasks.

Responsibilities

• Provide consultation and services to homebound individuals to address social and emotional problems impeding medical regimens.
• Support patients and families to navigate healthcare systems and care options.
• Collaborate with care teams to integrate social work into patient care and care planning.

Minimum Qualifications

• Master's Degree from a school of social work accredited by the Council on Social Work Education.

• State of MichiganFull Masters Social Work License Required

• Two (2) years of social work experience in health care setting.

• Ability to provide service to acutely and chronically ill patients

• Ability to complete written, verbal, and observational tasks indigenous to the position.
