Trinity Health At Home – Grand Haven is seeking a Hospice Medical Social Worker (MSW) to provide consultation and services to homebound patients, addressing complex social and emotional issues that can affect medical regimens.

Qualifications

• Master's Degree in Social Work from an accredited program.
• Licensed as a Master Social Worker in Michigan (LMSW/ LCSW).
• Minimum of 2 years in a healthcare setting.
• Ability to care for acutely and chronically ill patients.

Responsibilities

• Provide consultation and social work services to homebound patients.
• Support patients and families to resolve social and emotional issues.
• Collaborate with healthcare teams to optimize care plans.

Minimum Qualifications

• Master's Degree from a school of social work accredited by the Council on Social Work Education.
• State of Michigan Full Masters Social Work License Required
• Two (2) years of social work experience in health care setting.
• Ability to provide service to acutely and chronically ill patients
• Ability to complete written, verbal, and observational tasks indigenous to the position.
