Inova Loudoun Hospital in Leesburg, VA is seeking a dedicated Social Worker Case Manager to join the Case Management Team. The role evaluates patients' biopsychosocial needs and coordinates with physicians, nursing and the care team to ensure timely discharge planning and continuity of care across acute and post-acute settings.

The ideal candidate holds a Master of Social Work (MSW) and at least one year of clinical care or case management experience, plus Basic Life Support certification.

Qualifications

• 1 year of clinical care or case management experience
• Basic Life Support (BLS) certification

Responsabilités

• Assess patients' biopsychosocial needs and coordinate with the care team.
• Coordinate discharge planning and continuity of care across acute and post-acute settings.
• Serve as liaison between patients, families and interdisciplinary healthcare members.
• Document discharge planning information in medical records and communicate with care teams.
