Job summary

Inova Schar Cancer Institute is seeking a dedicated Social Worker Case Manager to join the Case Management Team. This full-time role runs Monday through Friday, 8:00am–4:30pm, with no evenings or weekends, and supports both Loudoun and Fairfax locations.

As part of the interdisciplinary team, you will assess biopsychosocial needs, facilitate care transitions, advocate for patients, and coordinate discharge planning and post-discharge services to ensure continuity of care within our

Qualifications

• Master's Degree in Social Work required.
• Minimum 1 year of experience in clinical care or clinical case management.
• Basic Life Support (BLS) certification required upon start.

Responsibilities

• Participates in assessment of patients' biopsychosocial needs and communicates with patients, families, and care team about care plans and progress toward treatment goals.
• Ensures options to support successful transitions and care plans are communicated and documented to ensure continuity of care; advocates to resolve barriers.
• Assesses psychosocial risk factors based on screenings and evaluates support systems and coping abilities.
• Intervenes with patients/families regarding emotional, social and financial consequences of illness.
• Provides counseling and interventions related to treatment and end-of-life decisions and advocates for patient empowerment.
• Delivers discharge planning and continuity of care for assigned patients in acute and post-acute settings.
• Initiates referrals to clinics, home health, hospice, SNF, rehab, and equipment as indicated.
• Collaborates with interdisciplinary team and families in discharge planning and post-discharge services.
