Ascension is seeking a dedicated Social Worker Case Manager for the Acute Rehab department at Elk Grove Village, IL. You will develop care plans, assess complex needs, and advocate for patient rights to ensure respectful, person-centered care.

You will maintain ongoing collaboration with leadership and care transition teams, supporting smooth transitions and ongoing reevaluation of plans. Eligible credentials CCMC/ACM preferred; Bachelor's in Social Work required; 3 years of experience preferred.

• Bachelor's degree in Social Work required.
• CCMC or ACM credential preferred.
• 3 years of experience preferred.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
  • None Required.
• Preferred Credential(s):
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC). ACM also accepted.
  • Certified Social Worker.

Education

• Bachelor's degree in Social Work required.

Work Experience

• 3 years of experience preferred.
