Job summary

Enriching Lives in Bakersfield, CA is seeking a Social Worker (Coordinator Emphasis) to oversee services for Individuals by developing and implementing the Individual Service Plan, coordinating with the Planning Team, and ensuring compliance with policies.

The role supports Family Home Providers, conducts trainings, and coordinates transportation, medical and related services, while maintaining documentation and crisis management coverage.

Qualifications

• Bachelor's Degree or equivalent combination of education and experience.
• Experience working with individuals with developmental disabilities preferred.
• Excellent computer skills and familiarity with Microsoft Office programs.
• Ability to read and interpret documents; write routine reports and correspondence.
• Ability to speak effectively before groups of customers or employers.

Responsibilities

• Oversees development and implementation of the Individual Service Plan with the Planning Team.
• Ensures provision of medical, health, and ancillary services as per ISP.
• Monitors Family Home Providers and supports service delivery.
• Provides crisis management support and rotates on-call coverage.
• Maintains documentation including ISP, progress notes, and incident reports.
