Job summary

Access TLC in California is hiring a BSW Social Worker/Case Manager to support HCBA participants in accessing healthcare, social services, and community supports. The role emphasizes coordination, advocacy, and education within a multidisciplinary team.

You will conduct psychosocial assessments, develop POT with the RN, and maintain detailed case notes in MedCompass, while ensuring privacy and regulatory compliance.

Qualifications

• BSW or higher with strong knowledge of social and functional issues in older adults.
• Experience in social services or case management preferred.
• Strong documentation and communication skills.
• Ability to work independently within a multidisciplinary team.
• Proficiency with Microsoft Office.

Responsibilities

• Collaborate with nurses, staff, and management to ensure DHCS compliance.
• Work directly with HCBA participants, families, and providers.
• Conduct psychosocial assessments and identify barriers.
• Develop individualized goals and connect with community resources.
• Coordinate with providers and IHSS for timely services.
• Maintain case notes and documentation in the MedCompass system.
• Conduct monthly case management calls and monitor wellbeing.
• Maintain confidentiality and comply with privacy regulations.
• Perform additional case management and administrative duties.
