Job summary

Under supervision, you will collaborate with a multidisciplinary team, deliver counseling, document thoroughly, and participate in discharge planning and care coordination, including home visits and community-based activities.

Qualifications

• Master’s in Social Work (MSW) is required.
• Bachelor’s degree in Social Work may be considered with relevant experience and language proficiency.
• Minimum of one year working with frail or elderly populations.

Responsibilities

• Provide psychosocial assessments and develop individualized care plans.
• Coordinate with caregivers and support systems to deliver coordinated care.
• Advocate, refer, and follow up on patient support in a timely manner.
• Offer individual counseling to patients as needed.
• Engage with community resources and conduct discharge planning.
• Document promptly and accurately, and participate in team meetings.
