Qualifications

• 2+ years experience in counseling, social work, or care coordination services.
• Familiar with managed care, population health, and discharge planning.
• Valid CA driver's license and reliable transportation.

Responsibilities

• Manage a caseload of PHM members focusing on behavioral health.
• Conduct assessments and develop care plans with members.
• Provide education to members on behavioral health and community resources.
• Document all encounters and outcomes in the Case Management system.
• Build relationships with community agencies for referrals.
