Job summary

Family Health Centers of San Diego seeks a Social Worker/Case Coordinator to help participants access Medi-Cal, CalFresh and housing resources while coordinating with the care team and community providers.

You will support transitions of care, attend team meetings, and maintain confidential records in a HIPAA-compliant environment. Travel across sites is required and a valid CA driver’s license is necessary.

Qualifications

• Bachelor’s degree in social work preferred and CPR/BLS or equivalent required.
• Travel between sites in San Diego County is required.
• Must have a valid California driver's license and car insurance.
• Bi-lingual English/Spanish preferred.

Responsibilities

• Assist participants with applications and access to community resources and social service programs.
• Serve as liaison between participants, families, caregivers and interdisciplinary team.
• Coordinate referrals and care transitions, including hospital and community placements.
• Maintain case records and documentation in compliance with regulations.
• Provide home and community visits as assigned.
