Job summary

Union Hospital Terre Haute, IN is seeking a Social Worker (SWR) to join the Population Health team. The role focuses on coordinating care, supporting patients and families, and collaborating with an interdisciplinary team to meet complex discharge needs.

The successful candidate will have an MSW or MS, be licensed or eligible for Indiana social work certification, and bring strong communication skills with a solid knowledge of clinical social work and state laws.

Qualifications

• Strong communication skills with excellent verbal and written skills.
• Solid knowledge of clinical social work and Indiana state laws.
• Experience in case management is preferred.
• Social Worker certification by the State of Indiana preferred.
• Five years of clinical experience within the last seven years.

Responsibilities

• Acts as liaison with the patient and family and the hospital.
• Collaborates with providers and interdisciplinary care teams.
• Reviews medical records and interviews patient/family to assess needs.
• Coordinates care plans including physical, financial, and psychosocial needs.
• Supports the Care Manager with complex discharge needs.
• Coordinates care for women’s and children’s services including adoption/guardianship.
• Maintains documentation in medical records and department systems.
• Supervises social work students as requested.
