Qualifications

• BSW required and Indiana licensure as a Social Worker.

• Strong interviewing, observation, and community resource knowledge.

Responsibilities

• Acts as liaison between the hospital, patient, family, and community agencies.
• Consults with the Social Services Coordinator on departmental activities.
• Uses varied communication methods and assesses their effectiveness.
• Maintains patient confidentiality and reviews medical records for needs assessment.
• Evaluates patient needs (physical, financial, psychosocial) and plans actions.
• Participates in meetings and serves on committees; engages in community activities.
• Develops social histories as ordered; supports licensure requirements and professional growth.
