Qualifications

• Master's degree in Social Work required.
• Permanent LSW licensure required.
• Crisis Prevention Intervention certification or similar de-escalation training within 6 months of hire.
• Knowledge of evidence-based psychosocial assessment, short-term therapeutic counseling, and crisis intervention.
• Knowledge of the disease process and human behavior in relation to health-related social needs.

Responsibilities

• Meets face-to-face with patients and families to assess needs and coordinate care.
• Communicates and collaborates with healthcare team to ensure safe, timely episode of care and provide resources and education.
• Interprets and communicates complex patient/family needs and models caring practices to other caregivers.
• Assesses psychosocial risk factors and evaluates support systems and coping abilities.
• Counsels patients and families regarding emotional, social, and financial consequences of illness.
• Accesses and mobilizes family/community resources to meet identified needs.
• Provides mental health assessments of patients upon physician referral.
• Provides therapeutic intervention to facilitate complex discharge planning.
• Serves as liaison with protective services and oversees abuse/neglect cases.
