Job summary

Union Health's Union Hospital in Terre Haute, IN, seeks a Social Worker MSW for Behavioral Health 4W. This PRN (on-call, day, varied) role provides comprehensive psychosocial care, timely interventions, and smooth transitions across the continuum of care; reports to the Director.

Qualified candidates will have a Bachelor’s in Social Work, at least five years of clinical experience, knowledge of Indiana state laws, and BLS certification.

Qualifications

• Bachelor’s degree in Social Work required
• Minimum of five years of clinical experience within the last seven years
• Strong knowledge of clinical social work practices and Indiana state laws
• Excellent verbal and written communication skills
• Experience in case management preferred
• Ability to manage complex cases and collaborate across interdisciplinary teams
• Strong organizational, documentation, and time management skills

Responsibilities

• Conduct comprehensive psychosocial assessments and identify complex needs
• Coordinate care planning and transitions across the continuum of care
• Collaborate with Clinical Care Coordinators, physicians, and care facilities
• Document care plans accurately in medical records
• Provide crisis intervention and safety planning as needed
• Lead transitions for complex patients and arrange transportation when required
