Job summary

Qualifications

• Bachelor of Science degree in Social Work and licensure as a Social Worker in Indiana required.
• Two to three years' social work experience would be helpful, particularly with health and crisis social work emphasized.
• Must have basic knowledge of human behavior.
• Must have good interviewing and observation skills.
• Must have knowledge of community resources and programs.
• Must have basic knowledge of medical and social work terminology.
• Must be able to relate to people and to think and work in an orderly fashion.
• Must have legible handwriting.
• Must keep aware of current thinking and trends in social work by reading literature and by attending appropriate meetings.

Responsibilities

• Acts as a liaison between the Hospital, the patient, the patient’s family, and community social and health care agencies.
• Consults with the Social Services Coordinator regarding departmental activities.
• Evaluates communication effectiveness and uses appropriate methods.
• exercises judgement when consulting with physicians, nurses, patients and families.
• Maintains patient and employee confidentiality.
• Reviews medical records, interviews patients or families as appropriate, assesses needs.
• Knows community agencies and resources for referrals.
• Participates in department meetings and committees as appointed.
• Engages in community activities and organizations.
• Assesses patient needs and develops action plans.
• Demonstrates understanding of patient and family needs across age groups.
• Initiates information and referral services to patients and families.
• Supervises social work students as requested.
• Contributes to staff development and quality improvement.
• Keeps licensure current through education and self-study.
