Job summary

PACE-RI Social Worker is a core member of the PACE Interdisciplinary Team. Working collaboratively with PCPs, RNs, other professional disciplines, and PACE participants, the Social Worker supports and advocates for the participants in achieving their health and social goals.

The social worker conducts assessments and provides education and psychosocial support for participants and caregivers of the PACE program.

Qualifications

• Master of Social Work degree required.
• 2 years of direct service experience required.
• Experience with case management and EMR systems (Cognify) preferred.

Responsibilities

• Provide orientation, information, and resources to participants and caregivers about PACE policies and services.
• Conduct biopsychosocial assessments and develop care plans.
• Coordinate respite stays, discharge planning, and care conferences.
• Maintain accurate documentation in the electronic health record within required timeframes.
• Liaise with IDT and care providers to ensure timely communication and service delivery.
