Job summary

Bluegrass Care Navigators is seeking a PACE Social Worker to provide and coordinate psychosocial services for participants and families, in the PACE Center, at home, or in facilities as needed. This role supports community-based case management for program participants and works with an interdisciplinary healthcare team to tailor care plans.

The position requires an MSW from a CSWE-accredited program and state licensure (CSW or LCSW), with at least two years in case management and experience

Qualifications

• MSW accredited by CSWE and licensure as CSW or LCSW
• Minimum of two years’ social service experience in case management
• At least one year experience with frail and elderly
• Strong networking with community resources
• Strong communication skills (face-to-face, phone, computer)
• Detail-oriented and organized
• Ability to interact with participants, families and agencies professionally
• Knowledge of interpersonal dynamics within an interdisciplinary healthcare team
• Ability to manage multiple priorities and handle emotional workload
• Ability to work independently with discretion and judgment
• Ability to access homes, hospitals, nursing homes and ALFs (stairs, spaces, climate)

Responsibilities

• Provide and coordinate psychosocial services for participants and families
