Job summary

Neighborhood Healthcare PACE is seeking a Social Services Support Specialist to provide non-clinical psychosocial support to participants in Riverside. The role focuses on coordinating community resources, documenting progress, and assisting families in navigating the PACE system.

It is an onsite, Monday–Friday 8am–5pm position with a focus on safety, well-being, and independence. The ideal candidate will have a Bachelor's or relevant Associate's degree with 1 year of social work experience

Qualifications

• Bachelor's degree required or equivalent experience in social/health field.
• 1 year social work experience preferred; experience with elderly population preferred.
• BLS certification required upon hire and maintained; CPR/First Aid required for all PACE employees.

Responsibilities

• Support MSW social workers by managing documentation and participant communications.
• Coordinate referrals to community resources (food, housing, utilities, caregiver support).
• Document progress in EMR and follow up on service referrals.
• Assist in organizing care team meetings and prepare notes.
• Support discharge planning by gathering paperwork and arranging transport.
• Maintain updated resource lists and contact logs for participants.
• Provide telephone and in-person support to participants and caregivers.
• Ensure HIPAA compliance and confidentiality.
