Qualifications

• Active licensure/certification as Licensed Clinical Counselor is required before hire date.
• Counselor credential from the Wisconsin Board is required before hire date.
• Social Worker credential from the Wisconsin Board is required before hire date.
• Case Manager credential from CCMC is preferred.
• Education options include high school equivalency + 2 years experience, an associate/technical degree, or 4 years of relevant experience.

• Inform & Document: Help shape post-discharge strategies and ensure precise documentation to align treatment options with patient needs.

What minimum requirements you’ll need

Licensure / Certification / Registration

• Required Credential(s): One or more of the following: Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
• Required Credential(s): Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
• Required Credential(s): Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.

• Preferred Credential(s): Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
