Job summary

Transcend Hospice seeks a Social Worker to provide coordinated home-based care across age groups, performing psychosocial assessments, counseling, and referrals to meet patient/family needs. The role reports to the Director of Nursing and requires a state-accredited SW degree, Texas certification, and a year of healthcare social work experience.

You will engage with interdisciplinary teams, assess psychosocial needs, and help families navigate eligibility and resources while upholding agency

Qualifications

Responsibilities

• Assess hospice patient to identify psychosocial, financial, environmental, and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
• Meet mandatory continuing education requirements of the agency/licensing board.
• Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
• Promote Agency philosophy and administrative policies to ensure quality of care.
• Attend interdisciplinary team meeting, participate in the patient care planning process and collaborates with Interdisciplinary Team to promote coordination of patient care.
• Accept clinical assignments that are consistent with the education and competence to meet the needs of the patients.

Job Classification: Clinical

Lines of Authority: Reports to Director of Nursing

Job Qualifications

Education: Bachelors or Master’s Degree in Social Work from a state accredited program. If has a Bachelor’s degree and is hired after 12/02/08, must be supervised by a Master’s of Social Work (§418.114).

Licensure: Texas Certification; Texas Driver’s License

Environmental and Working Conditions

Prolonged standing, walking, and sitting required; requires working under some stressful conditions to meet deadlines and patient/family needs; and to make quick decisions and resource acquisition; meet family/patient psychosocial needs.
