Wernersville State Hospital in Pennsylvania is seeking a Social Worker 1 to provide professional social work services to adults with mental health needs and to support them through treatment and recovery. You will assess, coordinate resources, participate in treatment planning, and assist with discharge planning to facilitate community reintegration.

The role is full-time (37.5 hours/week) with a standard daytime schedule, no telework, and requires a degree in social work or social welfare.

Formación

• Bachelor's or Master's degree in Social Work or Social Welfare required.
• Within 3 months of graduating with a qualifying degree may satisfy educational requirements.

Responsabilidades

• Service Coordination: Connect consumers with resources matching age, needs, and cultural background.
• Regulation Guidance: Interpret and apply federal, state, and agency requirements for proper service delivery.
• Assessment Work: Conduct interviews and gather information for psychosocial assessments.
• Treatment Team Support: Share updates, prepare documents, and participate in team meetings to guide treatment planning.
• Discharge Planning: Develop plans and coordinate with families and community partners for transitions back to the community.
