Qualifications

Responsibilities

• Assess patient needs by interviewing patients/families and reviewing records.
• Develop and implement psychosocial case plans with care team.
• Coordinate discharge planning and post-hospital resources.
• Provide crisis intervention and supportive counseling as needed.
• Deliver psychosocial counseling to restore optimal health outcomes.
