Job summary

Qualifications

Responsibilities

• Integrated Communication: Collaborate with patients, attorneys, courts, county staff, and treatment providers
• Assessment Coordination: Conduct psychosocial assessments, organize findings in the Electronic Health Record (EHR), and complete related forms
• Treatment Support: Participate in multidisciplinary team meetings, contribute recommendations, and update treatment plans
• Transportation Duties: Transport patients to interviews, community placements, and follow-up appointments
• Documentation Compliance: Maintain current records, meet regulatory standards, and ensure accuracy in treatment documentation
• Resource Collaboration: Utilize community resources, coordinate with outside providers to ensure continuity of care, and identify new resources as appropriate
