Job summary

The Grand Rehabilitation and Nursing at Queens is seeking a compassionate Social Worker to join our interdisciplinary team in Queens, NY. This full-time role focuses on psychosocial assessments, discharge planning, and resident advocacy to support families through the healthcare journey.

As part of our skilled nursing and rehabilitation setting, you will coordinate services, provide counseling, and collaborate with nursing, admissions, and medical staff to promote resident well-being and rights.

Qualifications

• Bachelor's degree in Social Work or related field required.
• Experience in Skilled Nursing Facility or healthcare setting preferred.
• Knowledge of discharge planning and CMS regulations preferred.

Responsibilities

• Complete psychosocial assessments for new admissions and ongoing resident evaluations.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Provide emotional support, counseling, and crisis intervention for residents and their families.
• Coordinate discharge planning to ensure safe and successful transitions.
• Assist residents and families with community resources, financial assistance, and advance care planning.
• Advocate for resident rights and promote dignity, independence, and quality of life.
• Participate in interdisciplinary care plan meetings and family conferences.
• Maintain accurate documentation in accordance with CMS, state, and facility requirements.
• Collaborate with nursing, rehabilitation, admissions, and medical staff to support resident-centered care.
• Assist with resident adjustment, conflict resolution, and grievance management.
• Support regulatory compliance and survey readiness.
