Job summary

Atrium Center for Rehabilitation and Nursing in Brooklyn, NY seeks a Social Worker to provide essential support, guidance, and advocacy for residents and families. The role focuses on interviews, discharge planning, and collaboration with an interdisciplinary team to improve quality of life.

The candidate should have a Bachelor’s degree (MSW preferred) and at least one year of supervised social work experience in healthcare; SNF experience is strongly preferred.

Qualifications

• Master’s Degree in Social Work (MSW) preferred.
• Bachelor’s degree required in Social Work, Sociology, Psychology, Special Education, Rehabilitation Counseling, or a related field.
• Minimum of one year of supervised social work experience in a healthcare setting, working directly with individuals.
• Experience in a Skilled Nursing Facility (SNF) strongly preferred.
• Strong interpersonal and communication skills with the ability to build trust and rapport.
• Compassionate, organized, and committed to improving residents’ quality of life.

Responsibilities

• Conduct resident and family interviews to develop social histories and individualized care plans.
• Partner with families to create programs that support residents’ emotional, social, and psychological well-being.
• Assist with discharge planning, financial resources, and coordination of community support services.
• Collaborate with an interdisciplinary team—including nursing, administration, and medical staff—to ensure holistic care.
• Advocate for residents by addressing social and emotional needs and maintaining clear, effective communication.
