Job summary

Woburn Rehabilitation and Nursing Center seeks a compassionate Social Worker to support residents and families in a skilled nursing and rehabilitation setting. The role includes psychosocial support, care coordination, and discharge planning working with an interdisciplinary team.

BSW is required; MSW preferred. Experience in SNF/rehab settings and knowledge of discharge planning and resident rights are valued.

Qualifications

• BSW is required; MSW preferred.
• Experience in skilled nursing facilities, long-term care, or rehabilitation settings is strongly preferred.
• Familiarity with discharge planning and community resources.
• Knowledge of resident rights and healthcare regulations.
• Strong communication, interpersonal, and documentation skills.
• Excellent organizational and time-management abilities.
• Ability to work effectively as part of an interdisciplinary team.

• Provide support and counseling to residents and families during adjustment to the facility and rehabilitation process.
• Lead and support discharge planning in coordination with hospitals, families, community resources, and other healthcare providers.

• Assist with admissions, transfers, and resident and family communication.
• Ensure timely and accurate documentation in accordance with facility policies and applicable state and federal guidelines.
• Participate in resident care plan meetings and follow up on assigned action items.
• Connect residents and families with appropriate community resources and support services.
• Advocate for resident rights, dignity, and quality of life.

Job Summary

Woburn Rehabilitation and Nursing Center is seeking a compassionate, organized, and dedicated Social Worker to join our team and support residents and families in a skilled nursing and rehabilitation setting.

The Social Worker will provide psychosocial support, assist with care coordination and discharge planning, and collaborate with residents, families, and the interdisciplinary team to promote quality of life and positive outcomes.

Responsibilities
