Job summary

Spring Grove Rehabilitation and Health Care Center is seeking a Licensed Social Worker to support residents with psychosocial care and care coordination. Must have SNF/LTC experience and hold CSW/LCSW/LSW credentials; a Bachelor's degree is required and a Master's is preferred.

• Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting required; experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.

• Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers.
• Build and maintain positive, supportive relationships with residents and their families.
• Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans for residents.
• Participate in care planning meetings and support continuity of care throughout the resident's stay.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with all applicable facility policies, regulations, and documentation standards.
