Job summary

Kaizen Lab Inc. is looking for a Social Worker to join our team at the Skilled Nursing Facility in Brooklyn, NY. The ideal candidate will ensure the emotional, medical, and social needs of residents are met while also advocating for them. You'll work closely with other staff and families for effective discharge planning and crisis intervention.

Qualifications

• Bachelor's degree in Social Work required.

Responsibilities

• Ensure that the medical, social, and emotional needs of each resident are provided for.
• Work together with other staff to identify potential difficulties for residents.
• Serve as a resident advocate.
• Provide crisis intervention, counseling, or other services to residents and families.
• Assist new residents and families in the initial adjustment phase.
• Work with residents and families on planning discharges.
• Act as a liaison to Federal, State, and local agencies providing service to the resident community.
• Maintain a productive working relationship with other department supervisors.
• Coordinate social services to assure the daily social services can be adequately provided.

Social Worker Responsibilities
