Job summary

Regal Care at Glen Ridge is seeking a Social Worker to support residents' emotional and psychosocial well-being in our skilled nursing and rehabilitation center. You will collaborate with residents, families, and the interdisciplinary team to address individual needs, provide counseling, and support quality of life.

The role emphasizes discharge planning coordination and maintaining residents’ rights in compliance with regulations; candidates should have a BSW and an active license, with MSW

Qualifications

• Bachelor’s Degree in Social Work (BSW) required; MSW preferred.
• Current Social Work license (LSW or LCSW) required.
• Strong interpersonal, communication, and organizational skills.

Responsibilities

• Conduct psychosocial assessments for new admissions and ongoing residents.
• Develop and implement individualized care plans in coordination with the interdisciplinary team.

• Serve as a liaison between residents, families, and facility staff.
• Document all assessments, interventions, and care plans accurately and timely.

Regal Care at Glen Ridge is currently seeking a Social Worker. The Social Worker will play a key role in ensuring the emotional and psychosocial well-being of residents. This position involves collaborating with residents, families, and interdisciplinary team members to address individual needs, provide counseling, coordinate discharge planning, and support residents’ overall quality of life.
