Job summary

Regal Care at Wakefield is seeking a Licensed Social Worker to support residents’ emotional and psychosocial well-being within a compassionate long-term care setting.

You will conduct psychosocial assessments, develop care plans with the interdisciplinary team, provide counseling, and assist with discharge planning to coordinate community resources and ensure residents’ quality of life.

Qualifications

• BSW required; MSW preferred.
• Current NJ Social Work license (LSW or LCSW) required.
• Experience in long-term care or sub-acute rehabilitation setting preferred.

Responsibilities

• Conduct psychosocial assessments for new admissions and ongoing residents.
• Develop and implement individualized care plans in coordination with the interdisciplinary team.
• Provide counseling and emotional support to residents and families.
• Assist with discharge planning and coordination of community resources and services.
• Serve as a liaison between residents, families, and facility staff.
• Document all assessments, interventions, and care plans accurately and timely.
• Advocate for residents’ rights and ensure compliance with state and federal regulations.

Regal Care at Wakafield is currently seeking a LicensedSocial Worker. The Social Worker will play a key role in ensuring the emotional and psychosocial well-being of residents.
