Job summary

Heart to Heart Hospice is seeking a Social Worker to assess patients, provide counseling, and coordinate community resources. You will address psychosocial, grief, financial, and environmental needs while guiding families through advance directives and funeral planning.

The role requires a degree in social work, a current state license, and at least one year of clinical social work experience, with hospice experience preferred. A company car may be provided for qualifying individuals.

Qualifications

• One year of experience as a Social Worker in a clinical care setting; hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs.
• Provide individual patient/family counseling, crisis intervention, and information on advance directives and funeral planning.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments consistent with education and competence.
• Perform on-call responsibilities and provide on-call service as assigned.
• Ensure timely submission of all documentation and compliance with laws and policies.

• Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
• One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred

What You Will Do:

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education andcompetence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
