Job summary

Heart to Heart Hospice is seeking a Social Worker to assess hospice patients and identify psychosocial, grief, financial, environmental and community needs, with findings documented in the patient care plan. You will provide individual counseling, crisis intervention, and guidance on advance directives, funeral planning and legal matters, and facilitate access to community resources.

A state social work license and a Bachelor's or Master’s degree are required, with at least one year of clinical

Qualifications

• Bachelor's or Master’s degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License, reliable transportation, and ongoing auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting; hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
