Job summary

Heart seeks a Social Worker in Seattle to assess hospice patients and provide psychosocial support, counseling, and resource coordination for families. You will document care plans, address grief and financial needs, and coordinate with the care team to optimize patient well‑being.

Responsibilities include on‑call coverage, timely documentation, and ensuring compliance with Medicare regulations. Requires a social work degree and current state license, plus a valid driver's license and reliable

Qualifications

• Bachelor's or Master’s degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License, reliable transportation, and ongoing auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting; hospice experience preferred.

• Submit documentation timely and accurately.
• Ensure compliance with local, state and federal laws and Medicare regulations.

What You Will Do:

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education andcompetence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
